One of the rough plot ideas that I have would be, there would be two main female characters and they each have their own love triangle issue because there's also two other main male characters that are their love interests. But maybe for one of the triangles the male sleeps with the woman that hates the female's guts which then would result in an accidental pregnancy that she claims to be his but is actually not but they don't find that out until after maybe a stillbirth? But he's in love with the main female character the entire time while struggling with his morals and obligations. Also, preferable premise would be hospital surgical residency.
